What Is CKD in Cats - And Why Does It *Almost Always* Get Caught Late?

If your cat has just been diagnosed with Chronic Kidney Disease (CKD), one question tends to surface before all the others: how did I not know?

The answer is not a failure of your love or attention: It is biology. And understanding it matters, because it changes how you think about every wellness decision for your cat going forward.

WHAT CKD ACTUALLY IS

Chronic Kidney Disease is not a single disease. It is a progressive, irreversible decline in kidney function that develops over months or years.

Healthy kidneys do far more than most people realise. They filter waste and toxins from the bloodstream, regulate blood pressure, maintain fluid and electrolyte balance, and produce the hormone that drives red blood cell production. When kidneys begin to fail, none of these functions collapse in isolation. The effects ripple across nearly every system in the body.

What makes CKD distinct from Acute Kidney Injury (AKI) is the word chronic. This is not something that happens suddenly. It is a slow, structural deterioration - most commonly the replacement of functional kidney tissue with scar tissue. Once that tissue is gone, it cannot regenerate. This is what makes CKD irreversible.

What is possible - and this is important - is slowing its progression, managing its complications, and protecting quality of life for far longer than most parents expect at the time of diagnosis.

CKD is one of the most common health conditions in senior cats. It affects an estimated 30–40% of cats over the age of 8, and up to 80% of cats over 15. If your cat is aging, this is not a remote possibility. It is one of the most likely challenges you will face together.

WHY CATS ARE BUILT TO HIDE IT

Here is the part that no amount of attentive parenting can fully overcome: cats are hardwired to conceal illness.

Please don't misunderstand this as stubbornness. It is survival instinct inherited from their wild ancestors. In the wild, a cat that displays vulnerability becomes a target. So cats evolved to mask pain and discomfort for as long as physically possible - and domestic cats carry this instinct completely intact.

By the time a cat appears unwell to even the most observant parent, the disease has often been developing for months, sometimes years. CKD is particularly well-suited to this kind of concealment. In its early stages, the remaining healthy kidney tissue works harder to compensate for what has been lost. The body adapts quietly. Your cat eats, plays, and grooms. You see nothing. And the disease continues.

This is not a gap in your awareness. It is a gap that biology creates deliberately.

WHY STANDARD BLOOD TESTS MISS IT TOO

Even when parents bring their cats in for routine wellness checks, CKD frequently goes undetected - and the reason is a limitation built into the most commonly used diagnostic marker.

For decades, the standard blood test for kidney function measured "serum creatinine" - a waste product that accumulates when kidneys can no longer filter it efficiently. Creatinine remains an important and widely used marker. But it has one critical flaw: creatinine levels do not rise meaningfully until a cat has already lost approximately 75% of functional kidney capacity.

By the time a routine blood panel flags a problem, 70–75% of kidney function may already be gone.

A newer biomarker called SDMA (Symmetric Dimethylarginine) has begun to change this. SDMA correlates closely with how well the kidneys are actually filtering, and its levels rise earlier - becoming detectable when roughly 40% of kidney function has been lost. This is still not "early", but it is a meaningfully different starting point for intervention.

If your cat is over seven and their senior wellness bloodwork does not include SDMA, it is worth raising with your vet. It will not catch CKD at its earliest, but it can shift the timeline of diagnosis in a direction that matters.

WHAT LATE DETECTION ACTUALLY COSTS

CKD has no cure. But the distance between early and late diagnosis is significant - sometimes measured in years of a good quality life.

A cat identified in the early stages can begin dietary changes that have been shown to slow disease progression. Blood pressure and protein loss through the kidneys can be managed before they accelerate damage. Monitoring can be calibrated. Decisions can be made from a position of time, not urgency.

A cat diagnosed later has fewer options: not because the management tools are different, but because the runway is shorter.

The most useful thing any medium-to-senior cat parent can do, regardless of diagnosis, is establish a baseline. Know your cat's normal weight, thirst, appetite, litter habits, and energy levels. Track them. Any meaningful shift deserves veterinary attention - not reassurance, not attribution to age.

For cats over seven, twice-yearly wellness checks including a full blood panel with SDMA, urinalysis, and blood pressure measurement are the closest thing to a genuine early warning system that currently exists.

CKD is not the end of your cat's story. But catching it early — or understanding why it wasn't caught early — is where every good decision from there begins.
